1. I consent to my child taking part in sport at the Club;
2. I am aware that, in some sports, my child may not always be supervised;
3. I understand that there is a risk of accidental injury to my child;
4. I understand that I will be kept fully informed of any illness or injury which my child suffers while playing sport at/in connection with the Club;
5. I consent to my child being photographed/videoed while playing sport and I consent to such photographs/videos being used in Club publications and/or on Club websites;
6. I have been made aware of the Club’s policies on changing and showering, managing children away from the Club, missing children, children playing in adult matches, anti-bullying and the wearing of appropriate equipment (information available on request from each Section of the Club);
7. I consent to my child travelling to away matches if selected for a Club team;
8. I consent to my child travelling in the vehicles of other players/parents where reasonably necessary;
9. Wherever possible, I agree to offer to use my own vehicle for Club/sporting purposes and I confirm that my M.O.T. and insurance are up-to-date;
10. Where I have given details of my child’s mobile telephone number I consent to that number being used as a means of contacting my child on Club related business;
11. Where I have given details of my child’s email address I consent to that address being used as a means of contacting my child on Club related business;
12. Where I have given details of my own mobile telephone number I consent to that number being used as a means of contacting me/my child on Club related business;
13. Where I have given details of my own email address I consent to that address being used as a means of contacting me/my child on Club related business;
14. I understand that I will be kept informed of sporting activities at the Club (e.g. timing and transport details for away matches involving my child)
